*Child's Immunization Records (per Colorado Law)
*Child's Birth Certificate
*Proof of Residence (i.e. Warranty Deed, Deed of Trust, Tax Notice, Lease Agreement, Assessor page, or notarized letter from the person you are living with)
We will have computers available for your convenience and look forward to meeting you!
